Clinical Utility: The Eurofins TRAC® (Transplant Rejection Allograft Check) donor-derived cell-free DNA (dd-cfDNA) assay is designed to utilize a noninvasive liquid biopsy to monitor the percentage of dd-cfDNA in transplant recipient plasma post-transplant using next generation sequencing (NGS).
Ordering Requirements:
- Pre-approved for outpatient renal clinic locations only. Orders from other locations must be reviewed and approved by the Laboratory Medicine Resident on-call.
- Providers must complete the Epic order questions with the following required information:
- Date of transplant
- Parent/Child Relationship (Yes/No)

Ordering & Collection

Collection Instructions:
- Refer to Eurofins TRAC Kidney Blood RNA Tube Specimen Collection Instructions.
- Use a butterfly collection needle. For best results, a 21G or 22G needle is suggested.
- Draw 10 mL whole blood in a Streck cell-free DNA BCT® collection tube (included in kit), filling the tube completely. If a kit is not provided, lab staff may use Streck tubes on hand.
- Gently invert tube 8-10 times to mix.
- Affix specimen label. Ensure that the labels do not obstruct the expiration date on the tube; it must be visible or the samples will be rejected.
- Approval Required
- Pre-approved for outpatient renal clinic locations only (UPOSKDN, RENALU, RENALH, TXCLIN). Orders from inpatient locations or from outpatient locations other than those listed must be reviewed and approved by the Laboratory Medicine Resident on-call.

Processing
- Receiving Instructions
Maintain and transport whole blood at ambient temperature.
Sendouts:
- Affix a Sunquest label to the Epic requisition (or Eurofins requisition, if included).
- Package the sample at ambient temperature according to kit instructions.
- Ship Monday - Friday via FedEx Priority Overnight. Samples sent on Friday must be marked for Saturday Delivery.
Reject Due To: Frozen whole blood, samples received >7 days after collection, specimen other than Streck Cell-Free DNA tube, patient identifiers on the collection tube do not match the requisition.
Whole blood in Streck cell-free DNA BCT® Tube, Stability: Ambient: 7 days; Refrigerated: Unacceptable; Frozen: Unacceptable.
