Ordering Recommendation: Most useful to establish autoimmune etiology in previously diagnosed type 1 DM. Do not use to differentiate type 1 DM from type 2 DM, for most cases. Many cases of autoimmune diabetes mellitus are associated with specific autoantibodies, including Insulin Antibody (Sendout) [RINSG], GAD65 Ab Assay (Sendout) [RGD65], IA-2 Antibody (Sendout) [RIA2A], and Zinc Transporter 8 Antibody (Sendout) [RZNT8A]. Presence and persistence of two or more of these autoantibodies predicts clinical DM. When evaluating for autoimmune diabetes mellitus, perform testing for at least two autoantibodies (typically, GAD65 in combination with another of the above autoantibody tests).
